Welcome New Hires!
It is my pleasure to welcome you to our team; we are excited to have you on board! Your first "test" as a new team member is simple: Can you follow directions? There is a lot to do before you are ready to start working. So, please follow these instructions very carefully.
Listed at the bottom of this page are several new hire forms that you will need to complete within 1 week from the date of your offer and return them to the Human Resources office. Some forms are optional, such as Direct Deposit; other forms only apply to certain age groups, such as the Work Permit application. All forms should be brought to Human Resources and reviewed before we schedule you for orientation. If you have any questions regarding the on-boarding process (including any paperwork) please use the following contact:

Here are the steps you need to take to begin working at Chick-fil-A Chicopee:
1. New Hire Paperwork
Complete All Hiring Paperwork that is located at the bottom of this page. Then contact HR to set up a time
to bring your completed paperwork in for review.
The following (a-d) must be completed.
a. I-9 and acceptable documentation
You will need to bring "acceptable documentation(s)" that you choose for completion of your I-9 form. To find out what is considered acceptable, please click here. We will need the original document(s) to complete the I-9 paperwork. You will receive the original document(s) back immediately. Note: Social Security Cards must be signed (if this is a form of documentation that you choose to provide).
b. W-4 and M-4
These are tax forms for Chick-fil-A Chicopee (i.e., Growing Servant Leaders, 24/6, LLC d/b/a Chick-fil-A Chicopee FSR) to know the correct federal and state income tax to withhold from your pay. If you do not reside in MA, please let HR know so they can issue you all of the proper tax withholding forms.
c. Work Permit
If you are under 18, you will need a Work Permit (see application below). This application needs to be signed-off by your school administrator. That administrator will then provide you with your official Work Permit. Please return your official Work Permit to the restaurant so that it can be added to your employee file.
d. Team Member Policy Handbook Signature Pages
You will need to print out the handbook signature pages after carefully reading through the handbook.
Please read the entire Handbook [Click Here] very carefully. The Handbook is password protected so you will need to request the password by texting or emailing HR before moving on. Please identify yourself in your correspondence.
e. Uniform Order Form
Please use either the Women's or Men's Uniform order form so that we can order you the proper fitting uniform.
2. Orientation Class
Once your paperwork is completed we will schedule you for Orientation. You must come to the orientation class on time and with all of your hiring paperwork fully completed. If you fail to show up on time or if you do not have all of your hiring paperwork, then you will have to wait until the next orientation class.
We use HotSchedules, the App or Website, to schedule you for your shifts. Once your paperwork is received, within a few days you will receive an email invite to HotSchedules. Once you receive this email, you will need to login and add your availability in HotSchedules so we can place you on the schedules. **Please review the availability contract prior to submitting your availability.
You will need to purchase work shoes that are (a) black and (b) labeled slip-resistant. If you show up to work without the proper shoes, you will be sent home.
Part of your hiring paperwork is a form titled: "Women's or Men's Uniform.". Please fill out this form to the best of your knowledge about sizes. We do have some uniforms you can try on for fitting purposes.
Once you have handed in this paperwork we will notify you that your uniform is ready for pickup, you can stop by and try on your uniform before taking it home with you. That way we can order different sizes if needed
